Workers' Compensation credentialing

The Society reviews and credentials Queens physicians who apply to treat patients under New York State Workers' Compensation.

  1. Apply online

    Create a profile and complete the required online training, about two hours. Paper applications are returned, and online applications can't be saved partway.

  2. Upload your documents

    • Current CV
    • Board registration, or proof of a completed 3-year residency
    • Two current letters of recommendation on letterhead, from a hospital Chief of Service or a comparable peer

  3. Choose your county

    Select Queens if you have a practice location here; otherwise, the county where your office is.

  4. Pay the processing fee

    By check payable to "Medical Society of the County of Queens", Attn: Lloyd Vance, or by credit card over the phone.
